I like to not phrase it as a question which seeks to establish guilt or blame. Rather, I approach the situation as an accident, and state “looks like we had an accident. who will be helping to clean it up?” I’m still there to guide. But the focus is on cleaning and the resolution, instead of blame and guilt. We all have accidents. Just take accountability and clean it up :)
