For the CSE, remember the guidelines thats the NBRC likes for the treatment of COPD and Asthma. They have very specific recommendations. For COPD exacerbation, 50-65 PaO2 is the recommended level, SpO2 88-92%. If they give you an option for heated high flow and the patient needs oxygen, thats usually the best answer (alternative is air entrainment mask, (nasal cannula never go above 4 liters for them.)) LABA/LAMA is the recommended treatment for outpatient/discharge, oral steroids are not recommended, inhaled steroids maybe in the outpatient setting. Asthma DOES recommend IV steroids but not inhaled or oral steroids. In asthma exacerbation, make sure you go through 3 doses of short acting bronchodilators first before choosing continuous treatment. Heliox therapy may be a good option in a severe case, can be given via high flow nasal cannula or nonrebreather. Outpatient asthma recommendation inhaled steroids for the MAINTENANCE of the disease, if the patient has complaints that its not working, recommend add on LABA/LAMA, and eventually a biologic agent like montelukast.
