@kneesportstherapist: ACL graft types... Over the years I've seen hamstring grafts, patella tendon grafts, quadriceps tendon grafts, allografts and more recently peroneus longus grafts. One thing that has really stood out to me from spending time in theatre is the surgical skill involved. There is a huge difference in how each graft is harvested, prepared and fixed into the knee. Different grafts behave differently, have different fixation methods and require different technical considerations. It's made me appreciate more than ever that you don't just want a surgeon who can do a graft. You want a surgeon who has truly mastered that graft. The "best" graft isn't the same for everyone. It depends on YOU.
Your knee.
Your sport.
Your goals.
Your injury history. One small red flag for me is a surgeon who only offers one graft option to everybody. The best surgeons I've worked alongside can discuss multiple graft choices and explain why one may suit you better than another. This isn't a post based on a study with 30 patients from 15 years ago. This is my take from daily clinical practice, seeing real patients, real outcomes and real grafts every single week. But honestly? Most people spend hours worrying about graft choice... Then spend the first few weeks after surgery lying on their back, waiting for a physio appointment and doing very little. The graft matters. But proper rehabilitation matters more. Without a structured rehab process based on progression, strength and testing (not just weeks post-op), your risk of a poor outcome or re-tear rises dramatically. I had my ACL and PCL reconstructed using Tibialis Anterior tendon (a tendon from the front of the shin/ankle area) and Semitendinosus tendon (one of the hamstring tendons). Would be interested in your thoughts on those.

Iv had patella graft , at day 30 and I’m just of crutches, quad was very hard to get going again I had no movement for the first 10days , got my self a tens machine and it has worked wonders , can now do straight leg weighted leg raises.. I don’t no about everyone else but I only exercise once a day but put 100% into it .. take me about 2 hours .. rather than 2-3 times a day ..

Quad graft is a living nightmare. It's big because you need all of it. The best research and results now is to avoid surgery and let the original ACL heal. No graft, no LET just rehab.
