PART 1: Inside a Real Arthritic Knee - The Anatomy of Degeneration Looking at a real human knee with severe osteoarthritis to understand what's actually happening inside your joint. ORIENTATION: This is a right knee - patella (kneecap) at the front, femur (thighbone) above, tibia and fibula below. Fun fact: the fibula is the lateral bone - that "la" helps you remember it's on the lateral side! THE LIGAMENTS: MCL (Medial Collateral Ligament): Broad, flat ligament on the inside of the knee. One of the most commonly injured. Attaches to the medial meniscus, so MCL tears can damage the meniscus too. LCL (Lateral Collateral Ligament): On the outside of the knee, prevents inward movement. PCL (Posterior Cruciate Ligament): Inside the joint, prevents the lower leg from sliding backward. ACL (Anterior Cruciate Ligament): The famous one! Prevents the lower leg from sliding forward and hyperextending. Forms a cross with the PCL (that's why they're called "cruciate"). LIGAMENTS DO MORE THAN CONNECT BONE TO BONE: They define the range of motion of your joint. The knee can flex, extend, and rotate slightly - but these ligaments keep everything stable and prevent movement beyond safe limits. THE DEGENERATION: Looking inside this knee, the articular cartilage is rough, non-uniform, and... blah (yes, that's the technical term). In a healthy knee, this cartilage would be smooth, glossy, and uniform across both femoral condyles. Listen closely as we tap: cartilage area = cushioned sound, bone-on-bone area = hard contact. That's severe osteoarthritis. Coming up: What causes this degeneration and how to prevent it.
