Not just that-- The bigger answer is anesthesia (unconsciousness) does not equal analgesia (pain relief). A patient can be completely unconscious yet still have large vital sign changes and even movement to painful stimuli under general anesthesia.

pilots maintain a sterile cockpit, i.e. no superfluous conversation during critical portions of flight. is this not a critical portion of surgery or is that concept not a thing? is this not a distraction?

local anaesthesia especially if mixed with adrenaline, has vasoconstrictive effects, thus helping with minimising bleeding during the surgical approach to the spine.
