I was working in the ER and chatting with co workers while I mixed up some Vancomycin. I scanned the med, but it wouldn’t scan. I turn it over to check the bottle…. It was VECURONIUM!!! I called the pharmacy and there were 5 more vials of Vecuronium in the slot that was supposed to have Vancomycin. I am SO thankful for the scanning process! It’s too easy to be distracted even by casual conversation and mistakes can easily happen. Scan your meds people!

I was a new nurse and there was Epinephrine where the Adenosine was supposed to be. Ours look similar. Pt was very unstable and HR in the 220s. I got it, connected it, almost pushed it but then looked down and realized it before I pushed it. Went back to the med machine and both drawers were completely mixed up

This happened just this past week. I’m an L&D RN and was about to give baby meds. I called the pharmacy multiple times because we were out of vitamin K injections. The tech came up to stock and handed me the injection. I took it into the room, went to scan it, and thought this looks wrong… she gave me an adult dose (10mg/mL) not an infant dose (1mg/mL)! I ran back and said, “do not stock this!”… she casually said, “it wouldn’t have let me scan it anyway…” GIRL! You just handed it to me!!!

We had a nurse who almost gave 1000 units of HumulinR instead of the prescribed 10 units, when doing a potassium shift. The mistake was caught by another nurse seconds before administration and of course it had to be reported to management.
