This is another example of a positive rooting response. He is three years old, non verbal, and presents with persistent drooling. The rooting reflex is a primitive reflex that should integrate by six months of age. The rooting reflex is controlled at the level of the brainstem, through the trigeminal and facial nerve pathways that govern the muscles of the mouth, lips, and jaw. In typical development, higher cortical centers come online and inhibit this reflex, taking over voluntary control of the same musculature. When that cortical takeover does not happen, the oral motor system stays under brainstem command. The child is operating the mouth through a newborn program in a three year old body. This has implications for speech. Intentional sound production requires precise coordination of the tongue, lips, jaw, and soft palate. A retained rooting reflex keeps these structures in a reactive, feeding oriented pattern. The tongue sits forward and low rather than elevating to the palate. The jaw destabilizes during attempts at articulation. The lips cannot achieve the movements needed for word formation. What presents as a speech delay is often a motor control problem rooted in incomplete brainstem integration. This is why drilling speech sounds and practicing lip closure often produces limited results in these children.
