Persistent redness and skin breakdown in the neck fold can be a sign of tension. Muscle tightness that keeps that fold compressed tighter than it should be, on one side more than the other. Which is also, not coincidentally, what torticollis looks like. So if your baby: — always tilts or turns their head the same direction — feeds better on one side — has a neck fold that's red, raw, or has ever bled That's not a hygiene issue.
