We often check B12 in people with chronic fatigue, expecting it to be low. But sometimes it comes back WAY above the upper limit of normal. I see this surprisingly often in people with active MCAS. I can’t prove that MCAS is causing it, but there’s an interesting connection: elevated B12 has been reported in a number of inflammatory states, including autoimmune and rheumatologic disease, chronic infections, inflammatory liver disease, and certain inflammatory blood disorders. One possible mechanism? Inflammation can increase B12-binding proteins such as haptocorrin, which can make the total B12 measured in the bloodstream look extremely high. So a B12 of 2,000+ doesn’t necessarily mean “B12 toxicity.” In someone who isn’t heavily supplementing, I sometimes view it as another potential clue that there may be a broader inflammatory process happening in the background—and that same process may be contributing to their fatigue.
