Hopefully this is changing as patients have access to systems like MyChart. Something that drives me crazy in visit notes is, “patient denies [insert symptom],” when I was never asked about it. That certainly seems like the doctor is a poor historian of that visit.

Chart notes can work in your favor though guys! If you have a medical professional refusing to order testing or try a treatment method you prefer- ask them to notate in the chart their refusal and the reason for it.

Thank you once again for an excellent video. I did not know the phrase “poor historian.” The problem with ordering your actual chart notes is that seeing what is written about you could be very upsetting. When I was in a wheelchair, I was unable to stand up and sit on the high exam table. In my chart notes, which they thought I would never see but I had ordered, it said that I was “uncooperative.” That was very upsetting to me.

This is why if you have a single bad experience with a first appointment, don’t go back, and go to another doctor and start over. There’s just no other way. If I have a bad experience in the first appointment, I won’t go back, I also don’t share records from that doctor that I had a bad experience with except to file complaints now. People need to file complaints as soon as you have a single bad experience the first time around. You file a complaint with a practice, you can file a complaint also with the state medical board and you can write a Google review. I do all of this now every time I have a bad experience the first appointment. They’ll keep doing it to other people if you don’t.

I hate they can label us , but we can’t do the same for the 7 minutes or the nurse who calls us back with results - I was told “ our bodies just do things” when I asked why something is happening - I always have a sheet of questions and my BP. Heart rate lately - at least I know my heart is ok this week after wearing a monitor and ultrasound. I guess chalk it up to anxiety?

My pain mgmt provider documents my visits into a Healow system that I can access. We have a great relationship, and she has been a life saver, but one time I noticed an inaccuracy in my notes. I spoke to her at my next visit and she made the change. It’s so important to stay on top of our documents/notes. Thank you for bringing attention to this!

I was a primary care scribe for years and am now a coder now who reads 20+ notes a day at work and a lot of documentation is wildly contradictive. I always review my notes and itemized bills for accuracy & there are usually errors, negligible and not really worth correcting but EHRs enable lazy charting. It is noted I’m an excellent historian though lol.
