@Jack Jones: The ending is that Charlotte, the nurse, did not kill Peter’s wife. Stephen becomes convinced that Charlotte murdered his mother and is trying to poison him and his younger brother, but this belief is the result of severe mental illness and paranoia. He begins hearing voices and interpreting ordinary events as evidence of a conspiracy.
In the climax, Stephen attacks Charlotte, believing he is protecting his family. He is prosecuted and convicted because the evidence shows Charlotte is innocent and Stephen’s actions were driven by his delusions rather than any real plot.
The story continues in the next episode, “Tina’s Story.” While serving his sentence in a young offenders’ institution, Stephen takes his own life. His father, Peter, is left devastated.
So the twist is that Stephen is wrong: the nurse is not a murderer. The tragedy is that grief and untreated psychosis convince him she is.
