Ok, I’m a carer in a dementia home. Notes are not done on a “ notebook” and left with her. The carer would have a proper either phone or tablet where they do their notes and this will be taken with them,due to confidentiality, not left with her. K has written this herself .

Most complex care places I’ve worked, for children and adults with severe epilepsy and disabilities we have actually sheet, specifically for them, medical sheets with body maps, obvs, if oxygen was needed and different sections you have to fill out for everything that happens before, during and after the seizure, that have to be filed and put in medical folders so it’s easily accessed to all caregivers through out the shifts and medical staff, ie hospital, ambulance, family. Not a random note book…

From working as a carer for 5 years not once did I ever write in a notebook🤣, it was all a4 sheets and that was only for creams, bath/shower records and fluid intake! Everything and I mean everything else was documented on a computer or an app on the phones provided from the place of work, we certainly didn’t write pages worth🤣

A carer / nurse wouldn't use a notebook as these could be altered? Pages torn out, words scribbled out etc? it would be all electronic wouldn't it?
I know many care homes use app based systems to relay information to family members, and K has stated many times she doesn't have access to her records as her mum keeps them, so why would a carer leave a notebook laying around?

Just want to point out as a carer who works as a PA and also through a company. You do have packages that use notebooks but they are solely in my experiance through a PA package which the family set up. PA’s can also be anyone (family,friends ect) and rarely have training as they are hired and paid for by the family. Any professionals would use proper documentation as it’s all recorded and in house documents don’t have detailed descriptions they go back to the office

wouldn't be a book the carer wrote in, only if the system is down it would be an A4 form with service users name, date, time, location, type of seizure, time started & ended, black ink only, care given,when the system back up it would be put onto her care plan on the computer, the note book could be used for simple staff communication but definitely not recording any seizures, proper forms must be used by law under Social Care Act 2008,

Also the front of the book is different the label she made was in the middle now it’s at the top and I bet it says her full name now not abbreviated she’s been crafting hasn’t she!

Hmmm there’s something not quite right about this book. There’s no dates/times on any entries. I would’ve thought carers would be using a tablet where they would have to log in, also I very much doubt a client/patient would be allowed to see what’s been written without going through the correct procedure to gain access to these notes. I really don’t believe carers are using a notebook and leaving it around for her to be showing it on TT.

I've never seen a carer's log writen like that, they are always folders with all info and pages for carer's to write about the shift, if something happens that notebook would not be official
