Ace says "you mean itd be better if I died" because he didnt think he was worthy of living, he probably thought he should have been in Sabos place and that Luffy deserved a better big brother, that he deserved to have Sabo, and he probably never let go of that grief, always thinking it would have been better had he been the one to die that day instead, because from a younge age Ace was told he didnt deserve to be alive, so much so those words embedded themselves in to his brain, twenty years he spent believing those words, and right when he started to think he truly had something to live for, that his life was meaningful, that he didnt deserve to die, he sacrificed himself to save Luffy, because hes a good big brother, and hed protect his younger brother no matter the circumstances
