@adytum_official: Mast cell activation syndrome (MCAS) is a recognized condition, with its own diagnostic code since 2016, in which mast cells (the immune cells behind allergic reactions) misfire and dump histamine and other chemicals at things that were never threats. The result is repeated episodes across multiple body systems: flushing, hives, gut cramping, a racing heart, lightheadedness, and a chemical surge that can feel exactly like a panic attack, which is part of why it gets mistaken for anxiety. It commonly travels with POTS and hypermobility. Why it goes missed for years: the main blood marker, tryptase, usually only rises for a few hours during an active flare, so outside that window it reads normal, and patients are rarely at a lab mid-flare. There’s also real disagreement among specialists about how to diagnose it, strict criteria require catching that tryptase rise plus symptoms in two or more organ systems plus a response to treatment, while looser definitions get criticized as too broad. So it sits in a frustrating gray zone. Honest caveats, because this one matters. MCAS is real and recognized, but its diagnostic criteria are genuinely debated, and not everyone with these symptoms has it. And to be very clear, this does not mean anxiety is fake or that everyone’s panic is secretly an allergy. Plenty of panic is panic. The point is that for a specific group of people, the ones with the hives, the gut symptoms, the trigger reactions, and the normal tests, it can be worth investigating mast cells too, while you keep working with your mental health providers. Ugh. My doctor sent me to an allergist who basically said I do not have this and sent me home. I have hives, tachycardia, bloating, ibs like symptoms, and hyper mobility. I don’t know how to get some one to take me seriously!

my mom and I were literally having this conversation today. I have hereditary alpha tryptasemia and I had to get it from someone. my mom's mom has severe panic attacks and couldn't leave the house after getting ready. I'm incredibly anxious and I only found out about MCAS bc I would have an allergic reaction to lip with red and yellow dye (I never wore lipstick prior) and were wondering if she was also allergic to the lipstick she applied right before leaving the house! hereditary alpha is also linked to retained primary dentition and both of us had that!

This can also be caused by Systemic Mastocytosis. Mast Cell Activation Syndrome has similar symptoms but SM is caused by a known genetic defect. Hematologists, allergists, and immunologists may be involved in diagnosing and treating either of those. There is another disorder of mast cells that can cause severe GI symptoms, Mastocytic Gastroenteritis. It can be caused by too many normal looking mast cells in the GI system. There are several medicines that can help symptoms of any of these 3 disorders.

Since being on ketotifen I’ve stopped having panic attacks, depression, anxiety. But it does cross the bb barrier so it kind of is in the head 🥰

Also in 2019 Psychology Today magazine wrote an article about how patients who are anxious should be tested. We make a different type of type. A 24 hr urine study is used to dx also a bone marrow biopsy if other tests are inconclusive

Allergies is every time I touch a cat my eyes get itchy. *Histamine INTOLERANCE is every time I drink red wine I get flushed and red and bloated(which 99% of TikTok says is mcas) . *MCAS-it doesn’t matter what diet changes or antihistamines i will RANDOMLY go anaphylactic. This is food/drink/smells/skincare/everything at random and NO PATTERN to it. It’s diagnosed and treated by an allergist but has a team of 7 doctors because yes they release histamine but also 200+ chemicals in your entire body. it’s nothing like intolerance or allergies.
