Having been an audio engineer for over 40 years, mixing the sound in the front of House sitting behind a mixing board in the audience and mixing the sound that the audience hears, any microphone would never be put underneath his shirt for two reasons first the shirt would mess with the frequency response of the microphone, and second any movement of his shirt would be picked up as a scratching sound through the sound system

It's totally normal to use a backup mic for some venues or for recording purposes... but inside the shirt makes no sense, Rode mics sound like shit inside a shirt. I've used the exact same mic doing events. It's supposed to be worn externally.

It was most likely designed to force the shape charge upward into his head. My guess is that due to his movements and the shifting of the magnetic mic, it instead went off in the direction of the neck area. Shape charge is exactly what it says, the force of the explosion (no need for a large explosive charge on this type of device) is designed to heat and push the copper shaped cone in a specific direction at a very fast and hot speed. They started using shape charge IEDs on us in Iraq, they are very devastating when they hit what they intended to hit.
