But how did she not notice that she wasn't attached or connected? That she didn't go through any part of that process?

complacency in adults is just purpose with unintentional consequences. if you failed to check the harness, you are guilt of murder. not manslaughter. murder. because you knew that without the harness, this person will die, then failure to check is on purpose. I know people don't understand or like logic, but everyone there is a murderer.

Also, in group situations, people often assume someone else did the important thing. Diffusion of responsibility? It is the reason why you point to 1 specific person in an emergency and designate them to call 911. Because people will assume someone else did.

it always bothers me when people go "well I would never do that if that happened to me" and it's like... you don't know because you haven't been in that situation. You really never know how you will react until you're actually in that stressful situation.

If only one person was responsible, this was less likely to have happened. There is a human tendency to think “that’s important, I’m sure the other got it”

The other comment I’ve seen is “I would never pay roadside bandits for a rope swing ride”. Which is a good position to have, because they don’t have a safety culture. But I’m lucky enough to know why a safety culture matters.

I was at an amusement park recently and every roller coaster and trill ride the people performing the safety checks on the restraints had a station and either had to give a thumbs up or on newer rides they pushed a green button. There were two people in the operating station and I believe one person was monitoring ride operations and the other monitoring the safety checks. If one person was out of place, one restraint not set, or one train wasn’t where it should have been, the ride didn’t start. I’m not sure they have a safety checklist but it really showed how each ride is ran as safely as possible

I have to admit I would definitely think of myself in the "I'd never do that" category. Good to be aware and more humble. Good advice ✌

Whenever I have to make changes after hours (computer stuff) that need to be in place for the start of business the next day, I usually make a little checklist for myself beforehand. There’s nothing like a pre-dawn call from the customer because you broke something with a missed step

The thing that gets me is the camera person appeared to notice beforehand and didn't speak up in time

the child in the car thing boggles my mind. if one thing changes in my routine, I start missing things. maybe it's just because I have ADHD, but in most of the stories I've read on kids being left in the car, it was almost always a day where there was a change in the routine.

My car alarms if anything is in the backseat when I open the driver's side door so you don't leave kids in the car.

HR here. Exactly. Thank you. I thought this same thing. I think there was no accountability or person in charge. They all were complacent.

when "everyone is watching the kids in the pool",, NO ONE is watching the kids in the pool. they all thought the other guy took care of it.

I’m a process consultant. This is an example of not following the process with tragic results. 😕

As a former rescue diver and rock climber, the safety checks are essential no matter how experienced. The sad thing is that the more experienced the more likely they skip the basics. Especially in rock climbing. So many pros have fallen to their deaths or severe injury b/c they thought they didn’t need to do the safety checks.

Professional scuba diver. I've jumped in the water without my fins or weights. More than once I've seen someone forget to zip up their drysuit. A flooded drysuit will drag you to the bottom and your death.

Sounds like the swiss cheese model for safety/redundancy. I did leave my daughter in my car in the summer when she was about a year old. Luckily, as I was walking into the store, it hit me. She'd been very quiet! That girl is 26 now.
