Why does insulin resistance make it harder to get pregnant? Most people think of insulin resistance as a blood sugar problem. But insulin also has a direct relationship with the reproductive system. When your cells become less responsive to insulin, the pancreas compensates by producing more of it. This means you can have relatively normal blood glucose while circulating insulin is already elevated. And this is where fertility becomes interesting. The ovaries have insulin receptors. Higher insulin can stimulate ovarian theca cells and amplify LH signalling, increasing ovarian androgen production. At the same time, insulin can reduce the liver’s production of sex hormone binding globulin (SHBG). With less SHBG available to bind testosterone, the proportion circulating in its free, biologically active form can increase. That combination of increased ovarian androgen production and higher free testosterone can interfere with normal follicular development. Instead of a follicle progressing towards dominance and ovulation, follicular development can become disrupted, contributing to irregular ovulation or anovulation. This is particularly relevant in PCOS, but you do not need to have PCOS for metabolic dysfunction to influence fertility. It’s also why I don’t look at fasting glucose in isolation. Someone can have a “normal” glucose result because their pancreas is producing more insulin to keep it there. I want to understand the metabolic environment surrounding ovulation, which can mean looking at fasting insulin, HbA1c and other metabolic markers alongside the broader clinical picture. And importantly, insulin resistance or PCOS does not mean you cannot get pregnant. Identifying what is disrupting ovulation gives us something we can actually work on.
