Chlorhexidine skin antisepsis - 2% chlorhexidine in alcohol for adults, pediatrics & neonates >2 wks. or >1500 gms - 2% aqueous chlorhexidine for neonates <2 wks. or <1500 gms 4. Optimal catheter site selection, with subclavian vein as the lowest risk of infection. Maintenance Bundle: 1- Rigorous Hand Hygiene: Perform meticulous hand hygiene, utilizing either an alcohol-based hand rub (ABHR) or conventional antiseptic soap and water, immediately before and after any vascular access device manipulation, dressing change, or insertion site palpation. 2- Aseptic Dressing Management & Site Care: Maintain a secure, intact barrier over the insertion site. Perform routine dressing replacements every 2 days for gauze dressings and every 7 days for semipermeable transparent dressings, or immediately if the dressing becomes damp, loosened, or visibly soiled. During each dressing change, decontaminate the skin with a >0.5\% chlorhexidine preparation with alcohol. 3- Mechanical Hub Disinfection: Mitigate the risk of intraluminal microbial contamination by aggressively scrubbing all access ports, needleless connectors, and hubs with an appropriate antiseptic (chlorhexidine, iodophor, or 70\% alcohol) prior to every fluid administration, medication delivery, or line access. 4- Daily Clinical Assessment of Line Necessity: Conduct a daily physical assessment of the catheter site via palpation and visual inspection to detect early signs of localized infection or tenderness. Evaluate the ongoing clinical indication for intravascular access daily, ensuring the immediate removal of any central venous catheter that is no longer strictly necessary for patient care. ##CLABSI##Public_Health
