Why do I always look at CRP and ferritin together? Because a ferritin result that looks “normal” doesn’t necessarily mean your iron stores are actually adequate. Ferritin is one of the key markers we use when assessing iron status because it reflects stored iron. But there’s an important piece of physiology that often gets missed: ferritin is also an acute phase reactant. That means ferritin can rise in response to inflammation. If someone has an infection, autoimmune disease, chronic inflammatory process or certain metabolic conditions, their ferritin may be pushed upwards even when their underlying iron stores are depleted. This is why I don’t like interpreting ferritin in isolation. CRP, or C-reactive protein, gives us information about inflammation occurring in the body. So imagine two patients both have a ferritin of 50 µg/L. Patient one has a low CRP. Patient two has significantly elevated CRP. Those two ferritin results cannot automatically be interpreted in exactly the same way. In the second patient, inflammation may be contributing to the ferritin concentration, potentially masking underlying iron deficiency. This is part of the reason iron studies become more complicated in inflammatory states. And there’s another layer here. Inflammation increases hepcidin, a hormone produced predominantly by the liver that regulates iron metabolism. Higher hepcidin reduces intestinal iron absorption and traps iron within storage cells, meaning less iron becomes available for processes such as red blood cell production. This is why, clinically, I’m not interested in asking: “Is ferritin inside the reference range?” I want to know what the entire pattern is telling me. Ferritin. CRP. Haemoglobin. MCV. Serum iron. Transferrin. Transferrin saturation. And importantly, what is happening clinically with the person sitting in front of me. Blood tests are not a collection of independent numbers. They interact, and context changes interpretation.
