You need to ask for the OR report which also includes the anesthesiology documentation that has all the medications that are given in the OR.

Get the ombudsman in your state on board with advocating for her to get the information that you guys need. There is also some sort of customer service liaison at hospitals that also helps bridge the gap of communication between providers and medical records.

Get the history and physical, ED notes both from the MD and triage nurse, progress notes hospitalist, progress notes specialist, op notes, MAR, discharge orders (may not be complete and not the same as a discharge summary) nursing notes, imaging reports, I do this five days a week.

I don’t work in the medical field but I believe you can ask for the operative report with transcripts. The last couple of procedures I had, the operative report literally included transcripts of the discussion in the room during the procedure. I don’t know if this is standard or just something my surgeons happened to do, but they provided a lot of information. I’m so glad your mom is doing ok and has you to help advocate for her during a scary time. 🫶

Hey Charlie. As a former patient advocate with five years of experience, here is what I would do Charlie if I was in your position. Go to medical records and get a copy of ALL of her records from the time she got into the ER until she was in ICU or wherever she went after surgery. Make sure you have a POA or a written authorization from your mom for you to get those records. Then I would reach out to the Patient Advocates at your mom’s hospital and ask to speak with the Chief of Surgery AND Anesthesia. You deserve answers and that is how I would do it.

Go to the patient advocate. They will create a report that will include the Patient Safety and Risk Management Department. Ask specifically for the Omnicell report. Those can not be manipulated-best of luck-

Anesthesia would be the one to talk to if possible. They were the ones who were giving the medications during surgery. Maybe you could specifically request their records and flow sheets from the surgery.

Narcs can cause obstructions also. I would want to know what caused the obstruction also. Patient advocate, ombudsman and lawyer.

You may also want to contact a patient representative at the hospital. They may also be able to help as far as making sure things were done right and to be sure her rights weren’t violated.

Yep, very similar story. You were saying you have an aunt that was in the medical field well, I would listen to that Everybody on this page will have something for you. That’s good. I don’t have any thing that I can offer you give you a prayer and also give your family a prayer. Don’t react in the moment give it a little time to process it. And make a decision.

This sounds almost exactly like what happened to my mom. She had a bowel obstruction and “code” on the table. Call the state to investigate this situation. As an RN I know what is happening. Have someone with her during the day. Get the OR charting

Hi, Your family is dealing with so much right now! There are so many things going on at once and staff will say different versions of what is happening, on top on just coming out of a huge surgery, stories may not seem consistent. Request a meeting w the surgeon and attending, have another family present to help interpret. Patient Advocates are available le to help you. The hospital wants you to feel satisfied, bit it may not feel or seem that way. In her medical records ask specifically for the operative and anesthesia notes, medication, diagnostic imaging and any consult notes. This should provide a better insite to what happened.

You should look up the 21st Century CURES Act and Information Blocking. You have the right to your Information. Blocking or delaying your access can result in significant fines for the hospital.

Hey friend! Sorry your mom is dealing with this. Her medical records won’t be closed out until after her admission. You can then request all medical records. Ask for medication logs, anesthesia charts and operative notes. Ask the surgeon what happened. Sometimes pain meds sneak up on people especially when people are in tons of pain. Anesthesia would have had an airway in surgery so I’m not sure what happened and never want to speculate. Hope she gets better soon. Praying for a full recovery ♥️
