Thats not even the true lore.. what is that larper talking about lol there are plenty of people that didnt flee. they just aren't in the game yet. They were in the original first few patches of zomboid but got removed because they were extremely buggy. When the game is finally ready for 1.0 and fully finished in 45 years there will be NPCs everywhere. So your comments are 100% true.

if the doors and windows are left open grime can accumulate, but that takes months/years. House closed up, it should stay relatively clean for quite awhile.

Don't know if this exists but it would be pretty neat if there was a mod where buildings are less dirty and some lawns are mowed and then as time progresses the Grass grows and buildings get progressively dirtier. if left for extended periods of time, it would add a level of upkeep to making a base look nice, but I think it would be a neat idea
