So an elevated anion gap indicates build up of ketones acids etc. so when the gap is closed ideally that is showing that those ketones or other byproducts are now decreased and back to normal levels within the blood showing that whatever treatment is being implemented is working?

First time having DKA pt the other day. My hospital uses beta-hydroxybutyrate as the gold standard instead of anion gap now. Super interesting!!

When I first started in the ICU and took a DKA patient, the nephrologist said my patient was no longer in DKA however the anion gap wasn’t closed yet. Then he told me to run a beta-hydroxybutyate. I said “sir please tell me you’ll put the order in because I don’t even know what you just said” 🤣 then I immediately did some research when the order came through. So glad you touched on anion gap because I was so confused as a new nurse why he thought it was resolved!
