Changing from a standard straight bar, to a single arm underhand variation, to a rope attachment changes the angle of the movement and can place more emphasis on different areas of the triceps. Each attachment creates a slightly different contraction pattern, which is why using multiple variations can help develop all three heads of the triceps more completely.
