I humbly request your kind attention regarding a serious issue with my academic records.
When I appeared for my SLC examination, I had not yet obtained my citizenship certificate. Therefore, my SLC certificate was issued with only my first name. Later, after obtaining my citizenship, I added my surname to my legal name.
However, during my Class 12 examination and later while obtaining my Tribhuvan University registration card, my name remained the same as it was in my SLC records. At the same time, both my SLC Character Certificate and College Character Certificate included my full name with the surname. My Citizenship Certificate, PAN Card, National ID (NID), firm registration, and all other official documents also carry my full name with the surname.
Furthermore, while studying BALLB, my transcripts were issued with my full name including the surname. Recently, after completing my Master’s examination, I found that the published result did not include my surname. Now, Tribhuvan University officials are refusing to issue my transcript, stating that my name does not match. I have been visiting the TU offices in Balkhu and Bhaktapur for the past two days, but I have not received any solution.
Sir, my registration number and academic records clearly belong to me. I respectfully request your intervention so that I can receive my Master’s transcript without unnecessary delay and hardship.
