If your knee won’t straighten, this is one deficit I wouldn’t just “wait out.” Especially after ACL surgery, restoring your normal extension early matters. But getting there isn’t only about stretching harder 👇 1️⃣ Activate the quad at end range Getting the knee straight passively is only half the job. Use terminal-extension isometrics and quad work to actively own those last degrees. 2️⃣ Use low-load extension work Heel props with gentle sustained loading can gradually expose the posterior structures to extension without turning every session into a fight. 3️⃣ Restore terminal-extension mechanics The tibiofemoral joint has to translate and rotate as you reach full extension. TKE variations can help integrate those mechanics under muscular control. 4️⃣ Address hamstring guarding After ACL injury or surgery, hamstring contracture can coexist with quad inhibition and hold the knee in flexion. Relaxation and quad activation may be more useful than simply pulling harder. 5️⃣ Carry it into your gait There’s little point gaining extension on the table if you immediately take thousands of steps with a flexed knee. Backward walking and gait drills can help reinforce terminal extension. 6️⃣ Low load + long duration Think frequent, tolerable exposure rather than one brutal mobility session. Consistency usually beats intensity. 7️⃣ Don’t ignore persistent extension loss Early extension deficits are associated with later extension problems. And if the knee feels mechanically blocked or simply isn’t progressing, get it assessed rather than endlessly forcing it. The progression: Gain extension → activate it → control it → walk with it → load it. And remember: 0° isn’t automatically “full extension.” If your other knee naturally hyperextends 5°, your goal may be to restore that individual baseline when appropriate.
