Sushruta is widely credited as one of the world’s earliest documented surgeons. Most evidence comes from the ancient medical text the Sushruta Samhita, traditionally dated between the late 1st millennium BCE and early centuries CE, though scholars debate the exact date. Plastic Surgery (Rhinoplasty) Sushruta is famous for describing nasal reconstruction surgery. In the Sushruta Samhita (Chikitsa Sthana 16), he explains how to rebuild a nose using a flap of skin from the cheek. This is among the earliest detailed surgical descriptions of plastic surgery known in world literature. The technique later influenced modern reconstructive surgery. Cataract Surgery The Sushruta Samhita describes a procedure called “couching” for cataracts, where a curved instrument was used to move the cloudy lens away from the visual axis. While not identical to modern cataract surgery, it is one of the earliest recorded eye operations. Surgical Instruments Sushruta catalogued about 120 surgical instruments and 300 surgical procedures. The Sushruta Samhita describes knives, forceps, needles, probes, and tubes, often modeled on animal forms. This represents one of the earliest systematic classifications of surgical tools. Human Anatomy through Dissection Unlike many ancient traditions, the Sushruta Samhita recommends careful study of the human body through cadaver examination. The text instructs students to observe anatomical structures directly, providing early evidence of empirical medical education. Fracture and Dislocation Treatment Sushruta described classifications of bone fractures and methods of reduction, splinting, and rehabilitation. The Sushruta Samhita outlines techniques for setting broken bones and treating joint dislocations, principles still fundamental in orthopedics. Medical Training and Ethics The text requires students to practice surgical techniques on fruits, vegetables, leather bags, and animal tissues before operating on patients. It also includes ethical rules for physicians, emphasizing competence, cleanliness, and patient welfare. Evidence from the Text The Sushruta Samhita states: “Only he who is skilled in both theory and practice is fit to treat patients.” This reflects its emphasis on practical training. Another passage instructs students to study anatomy through examination of a body, showing a commitment to observation rather than speculation. Historical Importance Modern historians generally credit Sushruta not with inventing all surgery, but with producing the earliest surviving comprehensive surgical textbook.
