Before we release a tongue tie… let’s see how baby is actually feeding. 👅🍼 A frenulum can be seen. But feeding function has to be assessed. A functional feeding assessment gives us a chance to look at the whole feeding picture—not just what’s happening underneath the tongue. We’re looking at things like: 👅 Tongue movement and oral motor function 🍼 Suck–swallow–breathe coordination 👄 Latch, seal and ability to maintain suction 💨 Clicking, leaking, gulping or excessive air intake 🤱 Milk transfer and feeding efficiency ⏱️ Endurance and fatigue throughout a feed 😣 Stress cues and baby’s regulation during feeding 🧠 Compensations baby may already be using to get the job done Because seeing a restricted frenulum doesn’t tell us the whole story. And a release is a procedure—not the entire treatment plan. Sometimes a functional assessment helps identify other factors that need to be addressed first or alongside a release. Sometimes it helps us establish a baseline so we know what actually changes afterward. And sometimes it helps a family ask better questions and make a more informed decision with their medical team. I’m not anti-release. I’m pro-function. I want to know what baby is struggling to do, how it’s affecting feeding, what we can support therapeutically, and whether additional providers need to be part of the conversation. The goal isn’t simply a tongue that looks less restricted.
