if you pay attention in the remake specifically you'll notice that every other human character (excluding maria who is a creation of the town) has a negative first interaction with James, Angela is startled and scared by him, Laura is hostile towards him, and Eddie is outright unnerved by him. Giving out this impression that James has a naturally off-putting presence hinting everyone sees him differently than he sees himself and by the end of the game when he accepts he's a monster, even the monsters of the town are scared of him, like he's been completely unmasked and they see him for what he really is.

I think what alot of people miss about this game is that it presents to you people that are on a moral spectrum. Eddie is entirely wrong for what he did, Angela is entirely justified in what she did, but James is a middle ground. He cared for his wife for 3 years, either directly or indirectly, he worked an office job to afford her care, but there was no getting better for Mary. She continued to deteriorate and suffer, so in a way what James did was a mercy, but it was still murder. In the true ending, he knows what he did and accepts his actions, but cannot handle the burden of having killed his wife, so he joins her in death by driving off a bridge into a deep lake.
