Racism can lead to the wrong diagnosis. Here’s how: A psychiatric exam involves interpreting behavior, affect, and speech in real time — and that interpretation isn’t neutral. The DSM-5-TR itself now includes a section on racism’s impact on diagnosis, and APA research has called the overdiagnosis of psychotic disorders in Black patients one of psychiatry’s most well-documented inequities — even when symptom presentation and demographics are controlled for. Meanwhile, mood disorders in the same population are often underdiagnosed. Culture and upbringing matter too. Idioms of distress, communication styles, and cultural context can be misread as pathology when clinicians aren’t trained to separate bias from data. To be clear: bias isn’t the whole story. Structural factors — discrimination, trauma, disadvantage — also drive real differences in mental health outcomes, independent of how a clinician interprets a symptom. Recent research (JAMA Psychiatry, 2024) backs this up. Bias and structural inequity operate together, not as competing explanations. Accuracy has to be the priority.
