Problem of the Day: Composite Diastema Closure Failure 🔴 The Problem: A patient presents with an anterior midline gap — a Diastema — and requests closure with direct composite. The dentist completes the procedure and the patient leaves satisfied — but weeks later they return complaining the teeth look strange, abnormally long or wide, or the composite has started discoloring at the edges. Diastema closure with direct composite appears deceptively simple — yet it is one of the most planning-intensive aesthetic procedures in dentistry. The biggest mistake is starting the composite placement without analyzing dental proportions — resulting in a final appearance that is disproportionate to the remaining teeth and face. Furthermore, closing a large Diastema with direct composite without prior orthodontic evaluation can create abnormal forces on adjacent teeth and accelerate restoration failure! ✅ The Solution: Step 1 — Assessment before execution: Identify the cause of the Diastema first: High Frenum attachment? Skeletal spacing? Sucking habit? Missing tooth? — Each cause requires a different solution. Composite is not always the answer! Silicone Index from the Wax-up: Provides the ready-made template for the final shape — place it over the tooth and pack composite from inside outward 6. Build the palatal wall first: Create the lingual wall with composite as a foundation shell, then build forward layer by layer — this gives you complete morphological control throughout 7. Optical layering technique: Dentin Shade at the depth + Enamel Shade on the surface — never use a single shade or the restoration will appear flat and artificial 8. Polish interproximal margins meticulously: Use Polishing Strips for all interproximal edges — these are the highest-risk zones for discoloration and plaque accumulation 💡 Tip of the Day: Before any Diastema Closure case — photograph the patient's full smile and sketch or digitally preview the expected result. Show the patient the anticipated outcome before starting and obtain their explicit approval of the proportions.
