@londononcologist: Some of my patients with stage 4 lung cancer have no evidence of cancer on their scans today. Here is why. What most people assume about stage 4: When people hear stage 4 lung cancer, they assume the goal is to manage it, slow it down, buy time. For some patients, that assumption is wrong. What is oligometastatic disease? Oligometastatic is a medical term for when your lung cancer has only spread to a small number of spots in the body, usually up to five sites. It is a subgroup of stage 4, and the rules for treating it are changing. What can be done? Instead of only treating the whole body with drug treatment, we target those specific spots to try and get rid of the cancer in those locations. We can use SABR, stereotactic radiotherapy, to the primary tumour and each site where it has spread. Why does this matter? If we can eliminate all the visible sites of disease, we give any prescribed drug treatment a better chance of working. In selected patients, the results can be really remarkable… no evidence of cancer on scans. I have a growing number of patients in my clinic who are years out from this treatment with no evidence of active disease on their scans. This approach is not standard everywhere. But with the ever changing landscape of cancer treatments it’s worth discussing. If you or a loved one have been diagnosed with stage 4 lung cancer and this has not been discussed, ask: "Do I have oligometastatic disease?” “Could targeted radiotherapy help in my situation?" That question could change the conversation entirely. My mother in law has non small cell. Single brain met removed, both adrenal glands removed and a lobe ectomy to remove primary tumour. 5 cycles of chemo, complete response. 5yrs clear pet scan.

My dad has nsclc metastasis to his peritoneal. All the tumors have shrunk but after radiation and the end of his immunotherapy cycles they’re still there.
