If you’ve spent any time online, you’ve probably seen claims that MTHFR mutations are the hidden cause of everything from anxiety and ADHD to autism, infertility, chronic fatigue, autoimmune disease, and more. The reality is much less dramatic—and much more interesting. Common MTHFR gene variants are incredibly common. Most people who have them are perfectly healthy. While certain variants can reduce the activity of an enzyme involved in folate metabolism and, in some cases, contribute to elevated homocysteine levels, the vast majority of claims circulating on social media go far beyond what the scientific evidence supports. That’s why most major medical organizations do not recommend routine MTHFR testing for common concerns like blood clots, recurrent pregnancy loss, or most chronic symptoms. In most cases, the results don’t change medical care. As physicians, our job isn’t to dismiss questions—it’s to answer them with evidence. Science is rarely as simple as a viral post or a 30-second video. Understanding what we know, what we don’t know, and where the evidence actually leads is how we make the best decisions for ourselves and our families.
