I’ve worked in many restaurants and have never seen the lettuce get washed or even rinsed.

I worked at the Olive Garden for 7 years, sadly, and they never did any of those things. The lettuce part of the salad came straight out of plastic bags. And the bread sticks come mostly precooked and are just browned a bit and are slathered with cheap margarine and sprinkled with garlic salt.
