As a Registered Nurse and Registered Midwife, I don't conduct every birth in the lying-on-the-back position. Whenever it's safe and appropriate, I also use squatting and hands-and-knees (kneeling/chest-down) positions during labor and birth. These positions can help some women feel more comfortable, use gravity to their advantage, and may help labor progress. The challenge is that many women are unfamiliar with these positions and may think you're doing something unusual. Also, while these positions can be more comfortable for the mother, they can make it more challenging for the midwife to visualize the perineum and assess the baby's descent compared with the lithotomy position. The most important thing is choosing the safest position for the mother and baby based on the clinical situation—not assuming one position is best for every birth.
