@doctor.painkiller: 10 Things You Need to Know About Botox for Migraine. For the right patient, Botox can be one of the best preventive treatments we have — but the key phrase is: the right patient. 1- Botox is FDA-approved for chronic migraine (15 or more headache days per month), not episodic migraine. 2- Mechanistically, Botox is not just “relaxing muscles.” In migraine, it works heavily through sensory nerves: calming pain signaling from overactive trigeminal/cervical nerve endings. 3- The results are not instant. I explain it as the “rule of 3s”: 3 days to start kicking in, 3 weeks to peak, 3 months before it wears off. 4- Studies show Botox can reduce migraine burden — frequency, severity, duration — and in most studies, patients see major improvement by the 6-month mark. 5- The needles are tiny and the whole procedure takes a couple minutes. In my experience, having done this procedure thousands of times- 95% of patients tolerate it extremely well. 6- The injection pattern is standardized — 31 sites — but it’s not always one-size-fits-all. For patients who also have TMJ dysfunction, we’ll sometimes add the masseter muscles, which can give an extra layer of benefit. 7- Common side effects are usually mild: a sore neck or some stiffness, mild bruising or tenderness at the injection sites. Most of it is short-lived. 8- And for anyone nervous about the cosmetic side: a lot of that comes down to the injector. A well-trained one knows how to avoid brow or eyelid droop, and the migraine pattern can be done in a way that targets the right muscles for pain relief without affecting your wrinkles at all. 9- The most common complaint I hear is that it wears off a little early. There are some small studies looking at whether zinc supplementation might help prolong the effect, but the evidence is very preliminary. 10- Approximately 47% of patients achieve a ≥50% reduction in migraine days in real-world practice. #migraine #migrainerelief

I wish it would be as easy as this. Getting insurance to cover even some of it is so difficult if not impossible

Hi, I'm a little confused. I've heard that migraine can't be seen on a MRI. If it doesn't show up on an MRI why would my scan say "Punctate clustered T2, T2 FLAIR hyper intense foci are seen in the left frontal deep white matter"?

Just had my first round of Botox yesterday. I’m hoping this finally helps with my migraines after trying other forms of treatment that didn’t work so well. 🤞🏻

Do you really recommend the TMJ injection when there’s data that says that it causes bone loss or is it just that maybe one or two injections would be enough to calm the situation? i’m giving us another go and I have TMJ and I can’t decide.

I’ve been on Botox for 11 years, it literally saved my life, I’d have 6-8 migraine free days a month. Now I still get them but not as severe and a lot less. I go every 90 days.

I finally got approved for Q10W injections. That 12 week wear off time was brutal. The entire last 10 days would be one long migraine.

For some reason Botox never gave me enough relief to be worth the money. I think it might be because I also have occipital and trigeminal neuralgia. Which kinda keeps me hurting all the time.

My first neuro that did Botox for me completely paralyzed my forehead/eyebrows and had eve lid drooping. Was told it was normal. New neuro does it and I have none of those effects…just symptom relief.

for the rule of 3s, is it concurrent or consecutive? i.e., is it 3 total elapsed months or 3 days + 3 weeks + 3 months?

What is the most affective placement locations for Botox? Do you inject into the muscle tissue or just under the skin?

I have hemiplegic migraines and Botox was my answer. I’m with that complaint that it wears off early. I am 15 days out from my next shots and the migraines are back. These 15 days are going to be rough.
