I just feel like the secret shop with Lann Tarv should be on this list. You gotta pass two skill checks in dialogue (assuming you pick the right options) to gain access to like 3 items. So many people miss it for sure!

DO NOT TRY THE SCYTHE, it only works in modded playthroughs. You need to be lvl 12 when facing him and ketheric doesnt show up so you cant get the netherstone softlocking the game.

Honestly on my first playthrough (durge) I accidentally hit the option to kill the cow at the inn. It caught me off guard that it turned into a slime thing.
