Text from my friends: Should I be freaking out about Cyclospora? My ID doctor answer: No. But if you have prolonged watery diarrhea this summer, I do need you to know what it is. Cyclospora is a parasite that causes an intestinal infection. It usually spreads through food or water contaminated with the parasite — and domestic outbreaks have often been linked to fresh produce. What we know right now: • State surveillance is where best monitoring is happening, but CDC reports 20 hospitalizations and 0 deaths across the 17 states affected.  • Michigan is now reporting a large outbreak, with 1,251 cases as of July 9 • No specific produce type, grower, supplier, or brand has been identified yet What it feels like: • Watery diarrhea • Cramping/bloating • Nausea • Fatigue • Loss of appetite • Sometimes low-grade fever or vomiting The clue: it often starts about a week after exposure and can last way longer than a typical viral gastroenteritis — sometimes days to weeks, with symptoms that improve and then come back. The good news: most healthy people recover, and it is treatable. The usual treatment is TMP-SMX/Bactrim.  Doctor-friend advice: If you have diarrhea that is persistent, severe, bloody, causing dehydration, or you are very young/older/immunocompromised/pregnant — please get medical care ASAP.  And if it has been going on for more than a few days, ask whether Cyclospora testing makes sense (Biofire panel is great if you have access to it).
