@doctor.atinuke: One of the biggest misconceptions I see in primary care is what an annual physical is actually designed to do. An annual physical is a preventive visit. If we also start evaluating new medical problems, that changes the type of visit. Sometimes we can address one or two concerns if time permits, but those concerns may be billed separately because they’re not considered part of the preventive service. I think the issue is that if we can’t talk about what problems have come up in the last year, what’s the point? May as well just keep on trucking until something breaks at these prices if all that happens at the physical is “you’re fine now give me money.”

This is why I’ve switched to concierge medicine/direct primary care. My doctor will address everything in one appointment. Waiting months between visits to discuss medical issues is why people are ending up with worse/more severe medical issues. How do you review overall health without hearing ALL of someone’s issues. You never know how or when these issues may be related.

Once again, you cannot assess wellness unless you have the full picture of everything your patient is experiencing. I believe what you mean to say is “ So sorry, but annual physicals are no longer designed to assess the patient and catch any emerging issues. Instead it’s 7- minutes of the dr staring into a computer wasting both their own time and their patient’s time.

This is a genuine question. I see a lot of these videos on TikTok and they’re always addressed at the patient, but then the comments blame insurance. Totally understand that insurance is the issue, but why are the videos never framed as, “This is how insurance limits the care I can provide you,” instead of as “patient education”? From my perspective, this framing only makes the tension between doctors and patients worse.

My current and last pcp always hears all concerns I have at my annual - new or existing issues. They're beholden to the same insurance standards so maybe you need to crowdsource among your doc community and see how they manage to accommodate the grey area instead of hardlining 🤷🏼♀️

I feel like the doctor then bears responsibility to educate the patient at the time of the visit and be like "we can talk about these things but just know and we're going to have to bill you separately for all of them.... so it's best if you just make separate appointments for each of your concerns. and I realize that you have a $60 copay with each of them and they're probably several months scheduled apart, and I know you already had to probably use PTO to come visit me but that's the way it is. sorry..."

So what is the point of a physical then? I understand these rules are made by insurance companies not you, but if I can't bring up any issues I don't really see how this visit helps me stay healthy since I have to lie to you. I already see my doctor regularly for chronic health issues so I have labs and other basic tests done on a regular basis and even then I'm lucky if the doctor actually gets up from behind the computer, makes eye contact, and performs even a basic physical exam.

I wish more people looked at their health and wellness the way we view behavioral health counseling - the lifelong issues we’ve carried aren’t solved in one visit with a therapist. It takes time, patience and regular visits.
