It’s a dogcheck. These usually appear when something went wrong with loading a room or when a save corrupts. It’s usually hard to recover a save if this shows up, so the best advice is to simply delete the save file and start over again.

So did you lose to Flowery and then die somewhere else? Or was it the secret boss? Both of those are known causes for getting a save file corruption (thta will fix itself once you reset don't worry)
