@erinrackham: Day 2–Guanfacine for hyperadrenergic POTS (.25 mg IR). I took .25 mg in the AM today as well as the PM because yesterday I got really dizzy and off-balance when the first pill wore off. 😳 That’s not happening today, thankfully! The effects are about the same as yesterday except I was more fatigued today. Wait. Do other people with hyperadrogenic pots also suffer chronic nightmares?! Mine are every single time I sleep, and they’re vicious. 😭 Would guanfacine take nightmares away?!??!

Hi I too have hyper POTS! I take 1mg extended release at night it took two weeks to adjust but it has been amazing for me! I still have moments of high heart rates but I haven’t gone above 145 since starting it! I hope it continues to work for you!

They had me try it with my ADHD before they would let me use a stimulant and I almost fell asleep driving home at night. It was awful! I hope you have success on it!

I am on day 2 going into night three of .5 mg once at night and I’m having a few negative side effects like constipation and bloating and headaches and dry mouth but my mind is slower and quieter, my focus is a bit better, and I feel much calmer and able to regulate my emotions better thus far. It’s cool to hear your experience so far too ☺️

I’ve been diagnosed with pots since 2020. I know that I prob got pots from covid and 6 years later they still can’t figure out what’s going on with my brain or heart 😅

I have the same type of POTS as you do. Guanfacine helps with adrenaline surges but gives me a headache and a weird kind of drowsiness. However, when I can’t handle the adrenaline, I have no choice but to take Guanfacine.

that's how it was when I first started taking it. but I stopped taking it a while back. i found my doctor didn't want to put me on the dosage I needed for it to actually work. so I stopped cold turkey that was bad lol but I've been great without it.

You'll eventually get to a steady state. Extended release will avoid the sedating peak and you can take it just once daily. .25 mg IR twice a daily is roughly equivalent to 1 mg ER once a day. "The ER formulation has reduced Cmax, delayed Tmax, and lower bioavailability compared to the same dose of IR guanfacine. Peak concentration of ER guanfacine is reached within 5 hours following dosing, whereas peak concentration of the IR formulation is reached within 1–4 hours following dosing ."

My fav thing about Guanfacine is that it helps with mindfulness - it’s like it allows me to stop and think and remember what I’ve already done! The sedation stops being an issue in my experience but you have to time your evenings carefully - if it peaks as you’re trying to sleep, your natural BP drops can clash with the Guanfacine drops 🥲

I had this type of pots secondary to EDS for 20+ years. Just a few months ago even after cutting my propranolol significantly due to very low HRs, they had to change my meds. My Dr gave me Diltizem 30 mg up to 4 times daily. I’m struggling because of the adrenaline. I’m shaking from the inside out multiple times a day. Scared they won’t find what works for me the way propranolol did 😭😩
