First, I would assess the patient and confirm the blood pressure by repeating the measurement manually if needed. Then I would assess for signs and symptoms of hypertensive emergency, such as severe headache, chest pain, shortness of breath, vision changes, or neurological deficits. I would continue monitoring vital signs, ensure IV access if needed, notify the physician immediately, administer antihypertensive medication only if prescribed, and document all assessments, interventions, and the patient's response

The simple truth:
A BP of 180/120 is not automatically a crisis. The real danger is whether it's already damaging organs (brain, heart, kidneys). Your very first job is to check for symptoms, not to treat the number.
Let get simple
Imagine a fire alarm goes off (the high number).
· Step 1: Do you see smoke or fire? (Symptoms like chest pain, stroke signs, confusion?)
· If YES fire: You call the fire department (IV meds in the hospital) and put it out slowly – too fast and the building collapses (stroke/heart attack).
· If NO fire: It's a false alarm. You don't spray water everywhere. You just check the batteries (re-check BP, adjust oral pills) and follow up later. And thank me letter thans
