@kathyzeleskey: I notify the referral source when the patient dies! It’s so simple, but very few people actually do it🤷♀️ I notify Everyone who was involved. Ex: For a patient leaving the hospital, that includes… Case manager, SW, MD who wrote the order, MD who saw patient in ER, every specialty who saw them in the hospital (those that make sense). And most importantly, their PCP. 😉 Little secret.. This is also one of the best ways to grow your territory organically. I’m a nursing home social worker in medical homecare sales but I agree with this! If something relevant happens with the patient after discharge I always update the referral source! Usually just a quick email but it help keeps the line of communication open and they like to stay updated!

Do you call them, write an email? I also prefer to notify most sources but have a hard time in a bigger hospital system vs clinic
