@kathyzeleskey: I notify the referral source when the patient dies! It’s so simple, but very few people actually do it🤷‍♀️ I notify Everyone who was involved. Ex: For a patient leaving the hospital, that includes… Case manager, SW, MD who wrote the order, MD who saw patient in ER, every specialty who saw them in the hospital (those that make sense). And most importantly, their PCP. 😉 Little secret.. This is also one of the best ways to grow your territory organically.
