So, I lived with 3 clinically diagnosed narcissists for over 27 years. I feel I understand them well. NPD is kind of like an addiction. They have no internal feedback loop. None. They are constantly seeking supply to basically exist. They operate like a focus group. They talk but they are always scanning the room for reactions to their behavior. Positive reaction = dopamine hit. Oooo, rinse and repeat. This quest for dopamine hits that they get no other way consumes them. Like any addict - they will do anything to get high. That’s where a huge part of their lack of empathy comes from - like any addict, they are plowing through to get a hit at any cost. Most addictions have a biological limit. You can only drink so much before you die, but for a narcissist EVERY interaction from buying a coffee to talking to your pastor is an opportunity for supply - I.e. a drug hit. It’s why the patterns are SO ingrained. It’s constant. Consequently when reactions are negative they feel shame - but it’s not shame like you or I feel. They feel ANNIHILATION. It’s like death, because in a way, it is.

The way that I got stared at when I would cry in the movies at an emotional scene “are you crying?!” Um yah I am

P.S :
It doesn't always appear the same way depending on the type of narcissist
