I think escape the backrooms is phobia based, it has the spider thing on level 188, that also has footsteps (Arachnophobia, phonophobia, Diokophobia) level 7 is literally called "Thalassophobia" level ! and !~! also include the fear of being chased, the fear of clowns is present on level fun, fun+ and 97, and the fear of puppets is also on level 97, the game is meant for people with fears.
