There are several serious problems with this message. First: tinnitus is NOT a "progressive degenerative disorder." (AT ALL!) Tinnitus is a heterogeneous symptom with hundreds of potential mechanisms and trajectories. By itself and without intervention or support it can remain stable, fluctuate, worsen, improve substantially, and even fully resolve. Describing it categorically as progressive and degenerative is scientifically incorrect. Second: hearing loss is not one single progressive degenerative disorder either. There are multiple forms of hearing loss, with different causes, mechanisms and trajectories. Third: hearing loss has been associated with an increased risk of cognitive decline and dementia, mostly in older populations. That is important science and deserves to be communicated accurately. But association DOES NOT EQUAL inevitability!!! The relationship is extremely complex, involves several pathways, as well as shared risk factors, and the evidence does by no means justify telling an individual patient, without assessment or context, a simplified story of: untreated hearing loss = reduced brain stimulation = tinnitus = cognitive decline and dementia Even the ACHIEVE randomised controlled trial found no significant reduction in three-year cognitive decline from hearing intervention across its overall population of 977 adults aged 70-84 with untreated hearing loss, although benefit was found in a prespecified subgroup at higher risk of cognitive decline. And then there is the context from which this SMS was sent... This message was sent BEFORE any personal contact with the patient had ever been established. Imagine receiving this shortly after developing tinnitus or hearing loss, when you are already frightened and desperately looking for answers, and you just filled out the contact form on an audiology clinic's website: "progressive" "degenerative" "cognitive decline" "dementia" followed by an announcement that you'll be evaluated and screened accordingly. We desperately need systems that help patients identify practitioners who communicate responsibly, understand the complexity of tinnitus, know the limits of their own discipline and collaborate across them. THIS is why I’m building the Phonisk Directory. Not another list of random providers who list "tinnitus" on their website. No, a full ecosystem designed around better triage, practitioner assessment, interdisciplinary care, referral pathways, education, and accountability. Patients deserve better than fear as an entry point into care. P.S. I will be reporting this clinic to the relevant regulatory body. #tinnitus
