NIGHT FLOAT RECAP⤵️ 12hr overnight shifts Mon through Fri for 4 weeks 👩🏼‍⚕️ I’ve been the doctor on call for any patient in the entire hospital experiencing a psychiatric emergency.  My main role is seeing patients in the emergency department in crisis, this usually means: a 30-min evaluation, reaching out to family members, coming to a diagnosis and writing up a report (note), starting (or de-prescribing) medications, and deciding if they need to be admitted to our locked psych unit, sometimes involuntarily. I’m often seeing patients and families on some of the worst days of their lives. I see psychosis, suicide, addiction, family conflict, homelessness and everything in between. This job can sometimes be discouraging because as a physician we can only do so much to try to help these patients, and I often struggle with being left feeling like there is no solution to address the overwhelming social determinants of health and lack of resources available for these vulnerable people. Overall I feel like I have grown immensely this month. At my residency program we don’t staff our decisions with an attending (unless we plan to discharge a patient who came in with a signed affidavit from a judge), so you really get comfortable making extremely difficult life or death decisions. Im entering the phase of residency where I truly feel like a confident doctor and it feels amazing. I hope I’ve made engaging with emergency mental health care less scary for patients who have had traumatic experiences seeking help in the past, and I go home every morning feeling like I truly made a difference in some of these patients lives.
