@ladyspinedoc: Replying to @TheActivityShift Case Study 237 - Thoracic Outlet Syndrome Your thoracic outlet is a narrow passage between the collarbone and first rib. Nerves and blood vessels squeeze through it every second of your life. When that space narrows (from posture, trauma, an extra rib, or repetitive overhead movement) you get pain, numbness, weakness, or circulation changes in the arm and hand. This case walks through diagnosis, imaging, and when surgery is actually indicated. #CaseStudy237 #ThoracicOutletSyndrome #Neurosurgery #SundayCaseStudy #ladyspinedoc

Thank you for talking about this!! I had bilateral TOS surgeries a few years ago, it was the best thing I ever did, but took YEARS to get diagnosed & was first sent to psych before getting it confirmed... bless you! 🤍

was that supposed to be really hard though to get that particular diagnosis?. it's not particularly hard if you have any actual education and knowledge in these areas. how about a case? that really is hard that even you, the specialists and the doctors rarely come across you like the 0.1 cases. the ones that really are hard, you know, perhaps do one of those cuz that'd be actually interesting and see how difficult it would be for the layman to catch it cuz I think perhaps you underestimate just how smart people are out there. so I'm curious cuz all I see a harder case and to know the outcome of the case as well. that really has benefited the patient and of course not yourself as a doctor cuz that's the important dynamic here. we keep forgetting. the patient is the one that comes first, not the doctor

What specialist is best to consult for this condition if one’s orthopedic spine surgeon is not comfortable in diagnosing ? Another orthopedic sub specialist? Neurologist? Vascular? Thoracic surgeon?

I have it and when I raise my arm, my finger tips got purple. One doctor say to me "it's easy, don't raise your arm" 😂😂

This condition is horrible. It’s nearly impossible to hold a job as everything requires functioning arms/hands. Treatment options aren’t great and it’s so difficult to get a diagnosis. There’s a massive lack of understanding in the medical field. I could go on and on about how much this condition can affect a person’s quality of life.

Roos test, not rose. Roos is a highly non specific test also. Almost every second person is positive for Roos test.

Over 10 years of misdiagnosis. Then I went to a brand spanking newly graduated osteopath. They were super excited - my symptoms matched an exam question 😅 My case is exactly as you presented. I had the 7th rib shortened. Symptoms pretty much gone (have some lingering issues due to long term nerve damage and fibrosis). Thank goodness for smart people. Very disappointed with the number of gps, specialists and physics who completely let me down.
