I've experienced number three and number five. It's funny how these promoter/manager/person who is supposed to pay you always seems to go home early or whatever. The worst example of this was a NYE gig I did in a very small town of Bremen GA. We were supposed to be paid by this guy who owned one of the largest car dealership dealerships in the area. We finished the gig, and this guy is nowhere to be found. He then gets in touch with the Band Leader and says "Man, I'm out of checks. I should be able to pay you in a couple of days." We did eventually get paid, but that was not the agreement that was in the contract. And you're absolutely right… Who hasn't had gear fail on stage? ALWAYS HAVE A BACKUP. I always take an extra snare drum, extra bass drum pedal and on bigger tours, I have an extra hihat stand & snare stand. I also always have extra heads handy. And there's no excuse for not having extra cables and if possible, an extra Guitar. You just never know.

When I was in college I worked two summers singing and dancing in one of those theme park review shows. Outdoor stage. Literally fell OFF the front of the stage one day into the bushes. 🤦🏼♀️. Just misjudged my steps dancing and WHOOP! Down I went. My favorite embarrassing moment.
