Sadly, many judge others before they are even licensed to practice or criticize a technique in which they are not skilled at. We need to look at and keep up with research but also understand that studies have their limitations and there is so much more to PT than just research studies. For example PTs who have done over 100,000 visits have certainly come across many cases where a technique works for a particular problem in a specific population or case that is deemed ineffective in a controlled study with 100 people not tailored to that specific situation. The same manual technique before, during and after a treatment may produce different results especially with the varying skill sets of clinicians. A procedure that may not be effective for 9 out of 10 cases can be a game changer for the 1 out of 10. A great PT knows when and how to use every tool in their tool box and understands the role and value of placebo.

I don’t believe this is true of all physical therapists, particularly those who understand what Sackett actually described as evidence-based practice. His model emphasizes the integration of the best available research evidence, clinical expertise, and the patient’s values and preferences; not research evidence alone.

Manual therapy is the best thing I can do, that no one else can do. Fix the muscles and fascia and then strengthen!!!!

Can’t wait to you to tackle insurance reimbursement, patient volume and mills, corporate healthcare spaces, weak national representation, excessive documentation, lack of upward mobility, and the like. So many problems with PT that this can be an ongoing series
