Iron bisglycinate or beef spleen. Which one should you actually take if your iron is low? They’re not interchangeable, and this is where I think a lot of the conversation around iron supplements becomes oversimplified. If someone has genuinely low iron stores, recurrent iron deficiency or significant losses from heavy menstrual bleeding, I generally want a therapeutic amount of elemental iron rather than relying on an organ supplement alone. One of my preferred forms is iron bisglycinate. Iron bisglycinate is a chelated form of iron that is generally well absorbed and tends to be better tolerated gastrointestinally than traditional forms such as ferrous sulphate. That matters because constipation, nausea and digestive upset are common reasons people stop taking their iron. But what about beef liver and beef spleen? This is where we need to distinguish between therapeutic iron replacement and nutritional support. Beef spleen naturally contains haem iron and can be a useful option for someone looking for more of a nutritional or maintenance approach. But if your ferritin is significantly depleted, you’re anaemic, you’re a very heavy bleeder or you have recurrent iron deficiency, I wouldn’t assume an organ supplement alone is enough to correct it. We need to consider the form, elemental iron dose, your blood results, how you’re taking it, what you’re taking it with and, most importantly, why your iron is low in the first place. I also commonly use alternate day iron dosing in appropriate patients. Iron absorption is influenced by hepcidin, so simply taking more and more iron doesn’t necessarily mean you’ll absorb more. Keep your iron away from coffee and tea around the time you take it too, as these can interfere with absorption.
